Membership Requirements

Personal Account

  • Proof of union or organization affiliation
  • Copy of valid ID (passport or driver’s license)
  • U.S. Social Security Number
  • Proof of address (copy of utility bill or lease agreement)
  • Minimum deposit of $100 in Savings (Share) Account (a Savings Account is required for all personal and business accounts)
  • Completed Membership Application Form (be sure to read both sides)
  • Read and agree to terms of “Important Account Information for Our Members” brochure

Member Application

Business Account

  • Copy of valid ID (passport or driver’s license)
  • U.S. Social Security Number
  • Copy of Certificate of Incorporation, Corporate By-Laws, or LLC
  • Copy of Taxpayer Identification Certificate or DBA Certificate
  • Proof of address (copy of utility bill or lease agreement)
  • Minimum deposit of $100 in Savings Account (a Savings Account is required for all personal and business accounts)
  • Completed Membership Application Form (be sure to read both sides)
  • Read and agree to terms of “Important Account Information for Our Members” brochure

Business Member Application

Child Performer and Coogan Trust Accounts

  • Completed application and Custodial Authorization Form
  • Copy of child’s birth certificate
  • Child’s U.S. Social Security Number
  • Copy of valid ID (passport or driver’s license of parent or legal guardian)
  • Parent or guardian’s U.S. Social Security Number
  • Completed Membership Application Form (be sure to read both sides)
  • Read and agree to terms of “Important Account Infomation for Our Members” brochure

Child Performer Custodial Authorization Form

Coogan Custodial Authorization Form

Payable-On-Death Account

  • Copy of valid ID (passport or driver’s license)
  • U.S. Social Security Number
  • Proof of address (copy of utility bill or lease agreement)
  • Beneficiary’s name, address, Social Security Number, telephone numbers, relationship to member
  • Minimum deposit of $100 in Savings (Share) Account (a Savings Account is required for all personal and business accounts)
  • Completed Membership Application Form (be sure to read both sides)
  • Read and agree to terms of “Important Account Information for Our Members” brochure
