For the fastest and most secure processing, please log into the Mobile App or Mobile Banking and fill out the Digital Debit Card Request Form.

Alternatively, you can complete and return the fillable Debit Card Request Application to Member Services. Please be sure to print, sign, and submit the completed form along with a copy of your identification. You may provide this form via email to Member Services at mservices@actorsfcu.com or by visiting one of our branch offices.
